■Note

  1. DO NOT use accents (e.g.  à, ç, ö).

  2. Enter the address you can receive Grade Report and Transcript of Academic Record after the dates below.

    Grade Report: after September 5 (Tue), 2017
    Transcript of Academic Record: after September 22 (Fri), 2017

    *We will send a photocopy of Grade Report and an original copy of Transcript of Academic Record to your home university after September 22 (Fri), 2017.
    *You do not need to register your home university's address here.

  3. If your address includes a room number, make sure to enter it; otherwise, the mail may not reach you.

  4. Personal information provided by you will be used only for the purpose of sending Grade Report and Transcript of Academic Record, or future contact.

  2. 成績通知表と成績証明書の送付先は、以下の日程以降に受け取ることが出来る住所を入力してください。

    成績通知表：　2017年9月5日(火)以降
    成績証明書：　2017年9月22日(金)以降

    *母国の大学へはこの登録とは別に成績通知表（コピー）と成績証明書を2017年9月22日(金)以降に送付します。
    *母国の大学の住所をここで登録する必要はありません。

  3. 住所に部屋番号がある場合は、入っていないと郵便が届きませんので、必ず部屋番号を入力してください。

  4. 登録していただいた個人情報は成績通知表・成績証明書送付および今後の連絡等に使用します。
