AY 2017 Hiyoshi Campus Bicycle Parking Lot Registration Form for Students Starting in Autumn

※A bicycle theft-prevention registration number is required for filling out this form.

利用期間：2017年10月2日〜2018年9月30日の駐輪場利用申請をする方は本フォームに入力してください。
入力後最下部の「内容確認画面へ」をクリックすると内容確認画面になります。
内容確認画面にて内容チェック後「事前エントリーする」をクリックしてください。
エントリー完了画面になり、管理番号と入力内容が表示されますので
必ず管理番号を控えておいてください。
（入力した内容および管理番号は連絡先メールアドレスにも送信されます）

Persons who will apply for use of the bicycle parking lot (period of use：October2th,2017〜September30th,2018)should fill out this registration form.
After inputting the required details, click on the “内容確認画面へ Confirm” button below to bring up the registration confirmation screen.
After checking the contents on this screen, click on the “登録する Register” button.
The registration complete screen will come up, displaying both the information you have filled in and your reference number.
Be sure to keep a record of these details.
(The information you filled in and reference number will also be sent to your contact e-mail address.)

本エントリーフォームの入力によって駐輪場の利用が確約されるものではありません。
期限を過ぎたエントリーは無効となります。
入力された連絡先は大学からの問い合わせやお知らせがある場合に利用されます。
